THE FOLLOWING DOCUMENTS MUST BE SHIPPED TO US
A signed passport containing at least 2 blank VISA pages. Your passport must be valid for at least 6 months from date of entry.
You must present a COPY of the bio information page of your passport to be included with your documentation.
1 (one) recent photograph, 2 inches x 2 inches, passport type.
China Consulates are very strict with their photo requirements:
- Photo must have been taken in the last 6 months
- Must be 2 inches by 2 inches in size
- Must have a white background
- You must be facing forward in your photo
- No glasses
- No jewelry
- No white shirts
- Mouth must be closed
Copy of flight itinerary. It should have your name, departure date, airline, flights itinerary showing entry and exit from each country.
Copy of both sides of your valid Driver's License or State ID.
A notarized copy of the front and back of the card ("Green Card") or notarized copy of U.S. visa in passport.
U.S. Visa - Visa that is in your current passport. If visa is in an old passport, the China Consulate requires the ACTUAL expired passport with the visa inside.
All applicants with criminal records and media professionals must apply in person at the Chinese Consulate in Chicago.
Any student visa applicants planning to stay for longer than 180 days must appear in person at the Chinese Consulate in Chicago.
JW202 Form: Only required for stays longer than 90 days. Please send the original JW201 or JW202 sent to you from school in China and a set of good quality photocopies. The JW201 or JW202 should have an official stamped on it. See Sample JW202 here.
You must obtain an original official letter of invitation from your sponsor. The organization you will be visiting is responsible for providing the invitation. This letter must:
- Specify the number of entries you are requesting for your visa
- Show your name exactly as it appears on your passport
A letter of acceptance from your Chinese based educational establishment is required. The Admission Notice should have an official stamp on it. Click here for Sample Admission Notice.
A notarized copy of the front and back of the Card ("Green Card") or notarized copy of U.S. visa in Passport.

- Any foreign passport holder carrying a J1 visa in their passport will have to provide notarized.

You are required to cover the cost of returning your Passport with Visa stamp. Please include postage payment or provide a self-addressed paid envelope for the delivery service desired (FedEx, UPS and USPS). Returning your package via the U.S. Postal Service is only acceptable with a tracking number.
